Appeals Process
Applicants who believe their application was wrongfully denied or wrongly awarded have the right to appeal the decision. The following forms – request for appeal and the appeals narrative – must be completed and submitted within 45 calendar days from the date of the decision.
Complete this request for appeals form in its entirety. Any documents related to and supporting the appeal must be attached to the appeal application for further review.
The narrative form will briefly describe the reason for the appeal and serve as the cover sheet to the Request for Appeal.

Required Documents For Bulk Payments
Bulk Payment Flat File
Landlords/utility providers must complete this flat file with the information collected from the attestation form and from internal delinquency records. The excel sheet contains steps on how to properly complete the flat file.
Fair Market Rents
The program will cover rent expenses up to the county’s fair market rents. View market rents.
FTP Uploads
DFA uses a secure file system to transfer documents between providers and the internal validation team. Landlords/utility providers need to upload the flat file(s) and attestations into the FTP folder.
